Assessment Tools

  • WAIS-IV – Intelligence adult

  • WISC-V – Intelligence children 6+

  • WRAT-V – Academic Achievement all ages

  • ABAS-III – Adaptive Functioning all ages

  • ADOS-II – Autism Related Behaviour Assessment aged 6+

  • SRS-2 - Social Responsiveness Scale

  • WMS-IV - Memory Scale

  • BRIEF-A & BRIEF-2 - Executive Functioning

Fees

  • The cost for one assessment is $2000

  • For multiple assessments, our fees start from $2500  

​The total cost includes a 3-hour assessment appointment, psychological report and feedback session.

​Assessment appointments occur on Tuesdays and Thursdays from 9:00am - 12:00pm.

Assessment Forms

  • Complete the four assessment forms emailed to you

  • Required one month prior to the assessment date

Deposit

  • Pay $100 assessment deposit

  • Required one month prior to the assessment date

Questionnaires

  • Please complete the questionnaires emailed to you.

  • Depending on the assessment you will receive an ABAS, SRS and / or BRIEF questionnaires

School & Healthcare Reports

  • Send school reports and healthcare reports to: reception@mindevolutioncentre.com or let us know if you’re dropping physical copies in prior to the assessment date

Day of Assessment

  • Please medicate you / your child as prescribed

  • If you / your child wears reading glasses, please bring these

  • Pay remainder of assessment fee
